How to use this report

You need a PC and Microsoft Excel version 2003 or higher to work with this report.

Send us a manual version

If you don't have Excel 2003 or higher, just print the report and enter the information manually. Then fax us a copy at (416) 327-6749. Remember to ask your principal to send a signed copy to the Student Success Leader at your board.

Send us an electronic version

If you have Microsoft Excel 2003 or higher, just complete the report in Excel and email it to us at studentengagement@ontario.ca. Remember to print a copy for your principal to sign and send to the Student Success Leader at your board.

If you see security messages while using the report, try changing the Excel macro security level to 'medium'. Here's how:

  • Click on 'Tools', 'Macro' and then on the 'Security Level' tab.
  • Choose 'Medium' and try opening the file again.
